Genealogy Research Form

Researchers should be aware that, while the Middletown Public Library has limited indexes of vital records, the records themselves are not kept at the library. (Please refer to the Genealogy page to learn where to get copies of the actual records.)

Research by e-mail is FREE, but we ask that customers pay for any photocopies made on their behalf. Currently copies are $.25 per page, with a $2.00 minimum charge, which covers both copies and postage.

Requests for research are answered in order of receipt. If you have not heard from us within two weeks, please feel free to contact us at 513-424-1251 to check on the status of your request.
